Submit Application

  1. Interest in becoming a medical doctor.

  2. Recommendations from science teacher and school counselor required (2).

  3. Letter showing attendance for previous and current school year.

  4. Passing score on grade level assessment & eligible for advancement.

5. High School and Middle School GPA 3.0 or higher

6. Essay- 500 word essay explaining why you want to be a doctor and how you plan on improving health equity.
